LIV Home Infusions does not store, manage, or control medical records.

  • coordination of care,

  • billing or payment,

  • or when required by law.

You have the right to:

  • access your medical record,

  • request amendments,

  • request restrictions,

PHI requests must be directed to the licensed medical provider via the Hydreight platform.
